We can make the most of your recovery if you come prepared to work...we can work hard to come up with a plan for successful return,
but we don't have a magic wand! Therapy recommendations frequently involve reduced training and lots of remedial exercise, but you'll
need to "buy into" the plan to make it work.
Some things to make sure you have:
· Physical therapy evaluation & treatment no longer require a prescription from your physician, however your insurance company may require one to pay for services. When you call to schedule, we'll always verify coverage, including the need to have a prescription. If you'll be coming from outside the Canton area, it would be best for you to fax a copy of the prescription ahead of time to be sure everything's OK (bring the original when you come for your appointment). Please remember that whatever we are told in verifying your coverage, it is your ultimate responsibility to arrange for reimbursement for services.
· If you have received treatment elsewhere, please bring a copy of any exercises you have been performing.
· Bring a swim suit. we may need to have you jump in our pool (rarely) as part of the assessment. It's also much easier to evaluate
female swimmers in a suit rather than yucky hospital-type gowns!

Make sure to download & complete a Swimmer's Injury History Form, complete it and bring it with you (you can also fax it ahead of time). You can download the Adobe Acrobat version from this page, or call us to have one faxed to you.Please plan to arrive 15-20 minutes before your appointment tome to allow for registration & changing. Many of our patients travel a significant distance for their care - Please leave extra time in your trip planning for traffic, construction and weather - our appointment times are blocked specifically for you, and late arrival will result in a shortened appointment so that we can care for the next scheduled patient.
